Double inlet ventricle. Lung biopsy findings and implications for management.

چکیده
The lung was biopsied in 20 children with double inlet ventricle and pulmonary hypertension aged 2 months to 14 years. Eleven patients had two patent atrioventricular valves, three atresia of the right valve, and six hypoplasia of the left valve. Severe pulmonary arterial medial hypertrophy occurred in the nine children less than 1 year of age. The findings did not suggest a period of normality after birth when the pulmonary artery might have been banded most effectively. Of the 11 older patients, eight had medial hypertrophy and three intimal proliferation with medial atrophy. Six patients with medial hypertrophy had some reduction in pulmonary arterial pressure after banding. It is recommended that the pulmonary artery be banded as early as possible, and rebanded early if a satisfactory result is not obtained, particularly in patients destined for a Fontan-Kreutzer procedure. Early atrial septectomy should reduce the arterial and venous abnormalities seen in left atrioventricular valve hypoplasia.

Double-Inlet Left Ventricle Before and After Septation
Septation is one of the surgical choices for double-inlet left ventricle, yet postoperative hemodynamics have not been well defined. We studied ventricular volume characteristics in 10 patients with double-inlet left ventricle before and after septation. Preoperative end-diastolic volume (EDV) of the ventricle was 291±111% (±SD) of normal and ejection fraction (EF) was 0.59± 0.07. Postoperative...
متن کاملDouble inlet right ventricle. Two pathological specimens with comments on embryology.
This paper describes two pathological specimens of congenital heart disease with double inlet right ventricle in which the ventricular chamber receives the right atrioventricular ostium and partially the left atrioventricular ostium, the two atrioventricular ostia being completely separated. The published reports are reviewed and only cases of double inlet left ventricle have been found. An ont...
متن کاملThe New Concept of Univentricular Heart
The concept of univentricular heart moved from hearts with only one ventricle connected with atria [double inlet ventricle or absent atrioventricular (AV) connection] to hearts not amenable to biventricular repair, namely hearts with two ventricles unable to sustain separately pulmonary and systemic circulations in sequence. In the latter definition, even hearts with one hypoplastic ventricle a...
متن کاملTwisted atrioventricular connections in double inlet right ventricle: evaluation by magnetic resonance imaging.
Twisted atrioventricular connections occur almost exclusively in the hearts with biventricular atrioventricular connections. Only one example of double inlet left ventricle has been illustrated in which the axes of the two atrioventricular valves crossed each other. We describe herein three patients, and one autopsied specimen, with double inlet right ventricle in which magnetic resonance imagi...
متن کاملIntracardiac anatomical relationships and potential for streaming in double inlet left ventricles
The aim of this study was to gain better understanding of the variable anatomical features of double inlet left ventricle hearts without cavopulmonary connection that would potentially facilitate favorable streaming. Thirty-nine post-mortem specimens of double inlet left ventricle without cavopulmonary connection were investigated. The focus was on anatomical characteristics that could influenc...
